You will use this template to insure nothing is left out. After you
complete the form, print it out and mail it to us along with you photos.
We prefer to receive all of our order forms and photos (be they prints or on
a disc or floppy) together in
one
envelop in the mail. Over time, we have learned it is less costly and
the projects stay more organized and are completed quickly if we follow this
method of coordination.

1.If
my layout requires photos, I will write my name and phone number as well
as the word "main" (for the main photo) on the back of my photo. If there
is a photo that most represents my vehicles color, I have indicated that
one as well. If I'm sending a couple, I will number them in the order I
like starting with number 1. Please remove the backgrounds
from all my photos.
Note:
If you need to view the Icon Library, Color Selection or Layouts pages
below after you've visited them, make sure to close their browser windows
(hit the X on the top right corner of your screen) instead of hitting the
back button. This way you will not lose everything you just typed.
(You will personally be printing
this form from your printer and mailing it in, so you don't worry about any
internet security problems.)

IF: I don't want my photos or finished Flash Boards used
in ads, brochures, illustrations, t-shirts or anything else that might make
me or my vehicle the least bit famous.
.I understand that all the photo(s) received by Flash Cards, Inc. are free of
any copyright issues and no copyright infringement is being violated by the
reproduction of the photos supplied. All photos will be returned in the
condition which they were received with the completed order. Allow 3-4 weeks
for orders. Cancellation of any order after boards are started is subject to
a $29.00 fee. Any returned checks will be subject to a $25 charge. No
C.O.D.'s. Although we do strive for accurate color, because of the nature of
ink jet printing, each board is printed for "pleasing color". Some boards,
however, may have slight color shifts to that of the supplied photos. Some
photos may have a slightly grainy and blurry appearance because of enlarging
so please send us sharp, crisp photos for best results.
